How To Get West Virginia Lpn License?

I am currently an LPN in Ohio and would like to get my license in the state of West Virginia. I have been on their website and can not find the information I need on how to apply. Can anyone please give some info?

    Try this website. Under Goals & Services click on
    II. Examination/Endorsement The forms that you need are there. You may need to retake the licensing examination or…at a minimum they will need to verify your license in Ohio. There might be criminal background check requirements and of course an accompanying fee.
